PERSONAL DATA: NOTICE AND CHOICE PRINCIPLES (S.7)
We wish to inform you that Mahkota Technologies Sdn Bhd, its subsidiaries and associated companies (“Mahkota”) currently undergoing an exercise of compliance of the requirements and regulations under the Personal Data Protection Act 2010. In line with this exercise, we are updating our database on the personal information of all Mahkota’s employees.
Your personal information that we previously collected from the below documents:-
- “Resumes” and the attachments such as any types of certificates,
- Details provided in the “Application For Employment Form” and/or
- Personnel Information Form.
Your personal information (“Personal Data”) which was disclosed for the purpose of employment or others, without limitation your:
- Name, home address (hometown, local and/or rented address), telephone number, place of birth, contact information including emergency number, next of kin information such as name and contact number, driving license that may be necessary for identification, security and contingency planning purpose;
- Age, gender, NRIC number/Passport number (or its equivalent), bank account number, spouse or partner information, dependant information, health information including but not limited to medical status, medical leave and other information that may be necessary for the administration of payroll, insurance, benefits and any other related issue;
- Education, training, work experience, employment history with or without referee informations, performance evaluations including performance records, promotion, talent planning, attendance record, references and background reports that may be necessary to establish, maintain or terminate the employment relationship;
- ‘Sensitive Personal Data’ such as marital status, race or ethnic origin, religious or other beliefs, criminal convictions, mental or physical health. Where we need to process such data; we will ensure that there is an appropriate legal reason for doing so, and that you consent to the processing. The only exception is where the processing of the data is necessary for the purpose of performing an obligation in connection with employment, which is imposed by law.
- Other information that may be necessary for legitimate business purposes or as required by applicable legal requirements; and/or
- Any other personal data that you may have provided for the purposes of your employment with Mahkota.
GENERAL PRINCIPLE
Mahkota may transfer, share and disclose your Personal Data to any of Company’s business partner local and overseas for business and performance purpose.
Your Personal Data may be disclosed to individual, companies, firm, corporation and government agencies for performance of contract of employment and reasons related thereto which shall include payroll process and statutory contribution, transfer and relocation, statutory requirement, incentive, back ground check, insurance, health, audit, reporting requirements with statutory bodies, tender and project requirements, law enforcement and compliance, judicial or administrative proceedings including the coordination and management of disciplinary actions and related investigations including merger and acquisition exercise. If we are contacted by any organizations or institutions, including banks or advocates and solicitors to verify your employment with us and your Personal Data thereto for commercial purposes, you hereby give consent to us to disclose the same.
Your Personal Data will not be disclosed to other entities except as provided in this Notice unless we are required to do by law or in the context of a merger and acquisition transaction. In the event that we engage external service providers, specific security and confidentiality safeguards have been put in place to ensure your personal data protection rights remain unaffected.
You may request for access to or correction of your Personal Data or limit the processing thereof at any time hereafter by submitting such request in writing to the respective personnel. Any inquiries or complaints with respect to your Personal data should also be channelled to us in this manner.
You represent and warrant that the prior consent of third parties (e.g. referees, family members, beneficiaries and emergency contacts) whose personal data you have provided during the employment application or service with Mahkota are obtained to allow us to do the same.
It is mandatory for you to inform us as soon as possible by filling up the “EMPLOYEE PARTICULARS UPDATE FORM” of any change to your personal details and ensure that it is accurate and up-to-date. The form shall be send to HCD Manager in a sealed envelope.
SCOPE
This notice is applies to all employees, regardless of position, department and location.
EFFECTIVE DATE
This notice is effective on Monday 10th February, 2014.
All consent letter(s) with the relevant form, if any shall be returned to HCD Manager in a sealed envelope latest by Friday 14 February 2014.
ACCESS PRINCIPLE AND CONTACT PERSON
Employees have the right to access/correct his/her Personal Data by filling up the respective form and send to respective personnel either by hand, mail or email at the following information:-

- Employee are responsible for advising the company promptly if any data is inaccurate, any changes in their personal data including the Sensitive Personal Data.
- Although not mandated by statute, employees are also advised to inform and updates the company of any additional academic or professional qualifications being awarded.
The respective form can be obtained from the above personnel.
SECURITY PRINCIPLE
Mahkota is responsible to take prudent steps to the confidentiality and security of all the Personal Data, including appropriate procedural, technical steps to protect Personal Data from accidental or unlawful destruction or accidental loss, alteration or disclosure. The steps may include entering into written agreement/form with who process Personal Data in accordance with Mahkota instructions and incorporating Mahkota’s own Data Protection standards as a minimum.
RETENTION PRINCIPLE
In executing its responsibilities with respect to the confidentiality of Personal Data, Mahkota will limit a number of personnel, appropriate to the sensitive of the information, to protect Personal Data against loss or theft, as well as unauthorised access, disclose, copying, use or modification. Such personnel will include physical and technological measures, for example locked filing cabinet, restricted access to offices, security clearance and limiting access on a “need to know” basis.
DATA INTEGRITY PRINCIPLE
Mahkota strives to maintain complete, current and accurate information about employees. Any inaccurate information shall be corrected as quickly as possible (provided information by employees, vice versa) after notification. Procedures will be maintained to ensure that any reported inaccuracies are promptly and effectively handled and that employees’ information remains as accurate, current and complete as possible.
For resigned employees, Mahkota will maintain all their Personal Data until 6 six years period. Such information must be kept in the office within the place of employment on which the employees are employed. Such information must be available for inspection by the relevant authorities and the employee.
Mahkota will imposed RM5 per/pc if document require include photocopy, image scan or document captured by any electronic device.
For unsuccessful employment candidates, the resume with or without attachment will be kept for 12 months period for potential employment, provided received after 15 November 2013. The previous resume whether in hardcopy and softcopy shall be deleted latest by 15 February 2014.
Failure to provide complete and correct information to Mahkota as required in the form and/or agreement or any addendum, among others may have effect on the :-
- Payroll;
- Insurance coverage;
- Employee appraisal and promotion;
- Correspondences and dissemination of information;
- Loan and/or leaning institution enrolment application; and
- Others.
DATA PERIBADI: PRINSIP NOTIS DAN PILIHAN (Seksyen 7)
Adalah dimaklumkan bahawa Mahkota Technologies Sdn Bhd, anak-anak syarikat serta syarikat berkaitan denganya (“Mahkota”) adalah di dalam proses pematuhan dan perlaksanaan peruntukan dna keperluan di bawah Akta Perlindungan Data Peribadi 2010. Sehubungan itu, pihak Mahkota juga di dalam proses mengemaskini data berhubung maklumat peribadi ke semua pekerja Mahkota.
Maklumat Peribadi anda sebelum ini telah diperolehi daripada dokumen-dokumen berikut:-
- “Resume”/Biodata berserta lampiran apa-apa sijil,
- Maklumat yang dinyatakan di dalam Borang Permohonan Pekerjaan, dan/atau 3. Borang Maklumat Kakitangan.
Maklumat bagi urusan permohonan pekerjaan atau lain-lain yang telah dizahirkan kepada pihak Mahkota (“Maklumat Peribadi”) antara lain adalah:-
- Nama, alamat rumah (alamat tetap, tempatan dan/atau alamat sewa), no. telefon, tempat lahir, maklumat termasuk talian kecemasan untuk dihubungi seterusnya maklumat keluarga seperti nama, nombor untuk dihubungi, jenis lesen memandu yang diperlukan sebagai pengenalan diri, keselamatan dan rujukan pada masa akan datang;
- Umur, jantina, No. Kad Pengenalan/No. Passport (atau setaraf dengannya), No. akaun bank, Pendapatan dan Elaun, maklumat pasangan dan/atau tanggungan, maklumat kesihatan tidak terhad status perubatan, cuti sakit dan maklumat lain yang perlu untuk urusan pembayaran gaji, insuran, faedah dan keuntungan, serta perkara-perkara berkaitan;
- Pendidikan, kursus/latihan, pengalaman kerja, maklumat pekerjaan lalu berserta/tanpa maklumat rujukan, penilaian prestasi termasuklah rekod prestasi, kenaikan pangkat, perancangan bakat dan kebolehan, rekod kehadiran, rujukan dan laporan latar belakang, yang difikirkan perlu untuk dibuktikan, dikekalkan atau ditamatkan;
- ‘Maklumat Data Sensitif’ seperti taraf perkahwinan, bangsa atau etnik asal, kepercayaan agama atau kepercayaan lain yang bersifat seumpamanya, kesalahan jenayah, kesihatan atau keadaan fizikal atau mental. Bila mana perkara tersebut diperlukan untuk memproses maklumat pekerja, kami akan memastikan bahawa ada sebab undang-undang yang sesuai untuk berbuat demikian, dan mendapat keizinan anda untuk memprosesnya. Pengecualian akan berlaku bila pemprosesan maklumat menjadi perlu bagi maksud melaksanakan obligasi yang berkaitan dengan pekerjaan dalam peruntukan undang-undang yang dibenarkan.
- Maklumat-maklumat lain yang mungkin diperlukan bagi tujuan perniagaan yang sah atau seperti yang dikehendaki oleh undang-undang terpakai; dan/atau;
- Apa-apa jenis Data Peribadi lain yang mungkin telah diperuntukkan bagi maksud pekerjaan bersama Mahkota.
PRINSIP AM
Mahkota berhak memindahkan Maklumat Peribadi anda kepada mana-mana rakan kongsi syarikat sama ada di dalam atau luar negara untuk tujuan perniagaan dan penilaian prestasi.
Maklumat Peribadi anda juga akan diberikan kepada individu tertentu, syarikat, firma, perbadanan dan agensi kerajaan untuk tujuan perlaksanaan kontrak pekerjaan, dan alasan-alasan lain yang berkaitan termasuklah bagi tujuan pemprosesan gaji dan caruman statutori, pemindahan dan penempatan semula, pemeriksaan latar belakang, insuran, kesihatan, audit, keperluan laporan kepada agensi berkanun, tender dan keperluan projek, perlaksanaan dan pematuhan undang-undang, kehakiman atau proses pentadbiran termasuklah penyelarasan dan pengurusan tindakan displin dan siasatan berkaitan. Sekiranya kami dihubungi oleh organisasi atau institusi termasuklah pihak bank atau firma guaman untuk mengesahkan perkhidmatan anda di Mahkota dan Maklumat Peribadi berkaitan untuk tujuan komersial, anda memberi keizinan bagi pendedahan tersebut.
Maklumat Peribadi anda tidak akan didedahkan kepada pihak lain kecuali yang diperuntukkan di dalam Notis ini melainkan ia perlukan oleh undang-undang dalam konteks transaksi penggabungan dan pengambilalihan. Sekiranya ia melibatkan pembekal khidmat luar, langkah-langkah keselamatan tertentu dan perlindungan kerahsiaan akan dipastikan dan dipatuhi sebagai langkah perlindungan Maklumat Peribadi tidak terjejas.
Anda dibenarkan untuk mengakses dan memperbetulkan Maklumat Peribadi atau menghadkan pemprosesan tersebut pada bila-bila masa dengan mengemukakan permohonan secara bertulis kepada kakitangan berkaitan. Sebarang pertanyaan atau aduan berhubung Maklumat Peribadi juga boleh disalurkan dengan cara ini.
Anda bersetuju dan mengaku bahawa maklumat pihak ketiga (seperti maklumat rujukan, ahli keluarga, orang yang mendapat faedah serta maklumat individu bagi kes-kes kecemasan), yang dibekalkan atau didedahkan kepada kami semasa permohonan jawatan atau semasa di dalam perkhidmatan telag diperolehi bagi membolehkan pihak Mahkota menggunakannya menurut keperluan.
Adalah menjadi satu kewajipan anda untuk memaklumkan kepada kami secepat yang mungkin dengan mengisi BORANG KEMASKINI MAKLUMAT PEKERJA/”EMPLOYEE PARTICULARS UPDATE FORM” untuk apa-apa perubahan Maklumat Peribadi dengan memastikan ianya adalah benar dan terkini. Borang tersebut hendaklah dihantar kepada Pengurus Pembangunan Modal Insan di dalam sampul surat bertutup.
SKOP
Notis ini adalah untuk semua pekerja, tidak mengira pangkat, unit dan lokasi.
TARIKH KUATKUASA
Notice ini berkuatkuasa pada hari Isnin 10 Februari, 2014.
Semua surat kebenaran bersama borang berkaitan, sekiranya ada hendaklah dikembalikan kepada Pengurus Pembangunan Modal Insan di dalam sampul surat bertutup, selewat-lewatnya pada hari Jumaat 14 Februari, 2014.
PRINSIP AKSES DAN KAKITANGAN UNTUK DIHUBUNGI
Pekerja mempunyai hak untuk mengakses/membetulkan Maklumat Peribadi dengan mengisi “Borang Kemaskini Maklumat Pekerja/Employee Particulars Update Form” dengan menghantar secara terus/serahan tangan, pos atau e-mel kepada alamat di bawah:-

- Adalah menjadi tanggungjawab pekerja untuk memastikan data yang dibekalkan adalah benar dan betul termasuklah di dalam Maklumat Data Sensitif.
- Walaupun tidak diwajibkan secara undang-undang, pekerja adalah dinasihatkan untuk memaklumkan kepada syarikat untuk sebarang maklumat termasuklah tambahan akademik atau apa-apa pencapaian yang diperolehi/dianugerahkan.
Borang berkaitan boleh diperolehi dari kakitangan di atas.
PRINSIP KESELAMATAN
Mahkota bertanggungjawab untuk mengambil langkah-langkah perlu untuk melindungi kerahsiaan dan keselamatan semua Maklumat Peribadi, termasuklah prosedur yang sesuai, langkah-langkah teknikal untuk melindungi Maklumat Peribadi daripada sebarang bentuk kemusnahan atau kehilangan sengaja atau tidak disengajakan yang menyalahi undang-undang, pengubahan atau pendedahan. Langkah
langkah tersebut termasuklah dibuat sama ada secara bertulis di dalam perjanjian/borang yang selaras dengan polisi Mahkota perhubung perlindungan Maklumat peribadi.
PRINSIP PENYIMPANAN
Bagi melaksanakan tanggungjawab berkenaan dengan kerahsian Maklumat Peribadi, Mahkota akan menghadkan kakitangan yang perlu berurusan dengannya, sesuai dengan tahap sensitiviti maklumat tersebut, bertujuan melindungi Maklumat Peribadi terebut daripada kehilangan atau kecurian, akses yangtidk dibenarkan, penzahiran, salinan dan mengguna atau mengubahnya. Langkah tersebut termasuklah langkah-langkah fizikal dan teknologi, sebagai contoh mengunci almari fail, akses kemasukan yang terhad ke pejabat, melepasi pemeriksaan keselamatan dan menghadkan akses hanya kepada perkara “perlu tahu”.
PRINSIP INTEGRITI DATA
Mahkota berusaha untuk mengekalkan maklumat yang tepat, lengkap dan terkini mengenai pekerja pekerjanya. Sebarang maklumat yang tidak tepat akan diperbetulkan secepat yang mungkin (maklumat yang diberikan oleh pekerja atau sebaliknya) selepas dimaklumkan. Langkah efektif akan diambil bagi memastikan sebarang ketidaktepatan dilaporkan dengan segera dan diperbetulkan secara efektif supaya maklumat pekerja kekal tepat, terkini dan lengkap.
Bagi pekerja yang telah tamat perkhidmatannya, Mahkota akan terus menyimpan semua Maklumat Peribadi sehingga tempoh 6 tahun. Maklumat berkenaan akan disimpan di tempat pekerja itu diambil bekerja. Maklumat berkenaan juga boleh diperiksa oleh pihak berkuasa yang berkenaan dan pekerja.
Mahkota akan mengenakan bayaran sebanyak RM5.00 bagi setiap helai dokomen sekiranya dokumen itu perlu dicetak, diimbas atau diambil menggunakan mana-mana peranti elektronik.
Bagi calon yang permohonan pekerjaan mereka tidak diterima, biodata bersama lampirannya sekiranya ada, akan disimpan untuk tempoh masa 12 bulan bagi peluang perkerjaan pada masa akan datang, dengan syarat ia diterima selepas tarikh 15 November 2013. Bagi permohonan yang lalu sama ada di dalam bentuk cetakan atau elektronik wajar dihapuskan selewat-lewatnya pada 15 February 2014.
Kegagalan untuk menyediakan dan membekalkan maklumat lengkap dan betul kepada Mahkota seperti yang dihekendaki di dalam borang dan/atau perjanjijan atau addendum (tambahan kepada perjanjian), akan memberi kesan kepada perkara-perkara berikut:-
- Urusan pembayaran Gaji;
- Perlindungan Insuran;
- Penilaian prestasi dan kenaikan pangkat;
- Kegagalan penyampaian surat menyurat dan maklumat;
- Pendaftaran permohonan pinjaman dan/atau institusi pembelajaran; dan
- Lain-lain perkara.
Job Summary
1. To actively attend project sites to assist the Project Manager in all civil aspects related to the construction of Substations.
2. To assist the Project Manager in all civil aspects up to the completion of projects.
3. To assist the division in tender preparation especially on the pricing of civil related e.g equipment and construction.
4. To implement all procedure and practices as stipulated in the Company’s policy, including ISO and Safety.
5. To provide good assistance before and during internal/external audit by ensuring all related documents at site is updated from time to time.
Duties and Responsibilities
1. To resolve site problems and working closely with Project Manager, Client, Supervisor/Engineer, site workers on all related parties.
2. To liaise with and resolve all civil issue with consultant/client.
3. To closely monitor site activities and ensure works are carried out as per plan and with highest quality/standard.
4. To calculate the required materials quantity to be purchased and ensure the materials are sent at site timely.
5. To prepare and check a progress claim and/or report to the client, complete with preparation if quantity taking off and always keep on until the progress claim is paid by the client.
6. To attend field and witness material tests and all related tests carried out as per required in the project/contract or specifications.
7. To prepare Method of Statement and any related technical required as per requested by client/consultant/company.
8. To attend Project Monthly Progress Meeting including Safety Meeting.
9. To prepare all site audit requirement and attend the audit and close any comment/NCR issued (if any).
10. To prepare and manage local authority’s submission, etc.
11. To provide calculation and pricing for civil portion of all tenders to be participate by the division.
12. To review pricing and equipment provided by sub contractors and supplier.
13. To update, maintain and achieve all related documents/manual/records/correspondences as required for ISO 9001:2015/45001:2018.
14. Ensure that all statutory requirements including Safety and Health etc. are fully complied as per OSH 18001:2007/45001:2018, etc.
15. Implementation of company procedures and practice at site.
16. Attend to auditor’s queries (internal/external).
17. To ensure organized filing system in order to have easy retrieval of all related documents.
18. Able to coordinate closely with all levels of staff to meet tight deadlines.
19. Build and maintain positive working relations with co-workers, relevant stakeholders and the public using principles of good customer service.
20. To perform any other relevant task as assigned by the Project Manager.
